Jade Fortes, LCSW
she/her/hers
I am a Licensed Clinical Social Worker (LCSW) with roots in both coastal Rhode Island and Washington, DC. I earned my Bachelor of Social Work at Salve Regina University and my Master of Social Work at Howard University. This background has shaped my perspective and continues to guide how I support individuals through meaningful life experiences. My work focuses on supporting individuals through medical and mental health diagnoses, significant life transitions, and experiences that reshape identity and independence. I work with individuals and families across the lifespan, creating a supportive space to process change, regain steadiness, and move forward in ways that feel manageable and grounded.
My clinical perspective is shaped by work supporting individuals through significant life experiences, periods of change, medical and mental health diagnoses, grief, and changes in independence, along with experience in community crisis stabilization, therapeutic residential treatment with adolescents, and memory care services. Working with individuals and families through complex diagnoses, cognitive changes, and high-stress transitions has deepened my understanding of how emotional wellbeing connects to health, environment, and sense of self. This foundation informs a steady, thoughtful, and genuine approach to care.
In my work, I draw from Cognitive Behavioral Therapy, narrative therapy, strengths-based approaches, and mindfulness practices. I focus on building insight while offering practical, accessible tools that support lasting stability and a sense of direction. Outside of therapy, you can usually find me enjoying a good movie or TV show, spending time with my two nieces, or at the gym pretending like I know what I am doing.
